A nonprofit volunteer agency that provides information and referral services to those afflicted with Alzheimer's Disease, their caregivers and families. The Chapter also holds monthly family support group meetings. Fees:  None Eligibility:  Persons afflicted with Alzheimer's Disease, their caregivers and families.

A help line for seniors, caregivers and persons living with a physical disability needing information and assistance in accessing a myriad of services available to them.   The ADRC provides information on a broad range of programs and services, helps people understand the various long term care options available to them, helps people apply for programs and benefits, and serves as the access point for publicly-funded long term care.  These services can be provided at the ADRC, via telephone, or through a home visit, whichever is more convenient to the individual seeking help. Photo ID cards are issued to seniors 60 years of age and older.  This ID entitles the bearer to various discount privileges with local merchants.

Free Wills Program

Outreach to the Elderly
This program seeks out individuals who are unable to actively seek help for themselves. It helps seniors solve problems and figure out solutions. Home visits and satellite locations offer seniors access to information and assistance and linkages to services to enhance their quality of life.

Global Options for Long Term Care Global Options for Long Term Care are Medicaid Waiver Programs (income and clinical eligibility required) that offer several care alternatives to individuals otherwise at risk of placement in a nursing facility. These programs offer opportunities to receive individually tailored services in the community setting of their choice.

In-Home Components: Global Options for Long Term Care (GO) Provides in-home services to seniors at risk of placement in a nursing facility. GO is designed to supplement the assistance an individual receives from his/her natural support network of family, friends, and neighbors, as well as from community agencies and volunteer groups. By designing service plans uniquely tailored to the individual, GO works to enhance the individual's community care options. GO also strengthens the ability of caregivers to continue in their vital role as primary support providers. Application for GO is a two-part process. First, financial eligibility is determined; second, functional eligibility is evaluated.

    GO Eligibility Criteria:
  • A New Jersey resident age 65 and older, or 21 to 64 and disabled as determined by the Social Security Administration
  • Requires a nursing facility level of care but wishes to remain at home
  • Financially eligible for Medicaid or Medicaid Waiver Services
  • US Citizen or a Qualified Alien
  • Has no alternate means to obtain the needed support
  • Resides in home or apartment that is not a licensed residence

Residential Component Assisted Living Residence (ALR) Is a residential facility licensed by the Department of Health and Senior Services to provide apartment-style living and congregate dining and to assure that assisted living services are available when needed. Apartment units offer, at a minimum, one unfurnished room, a private bathroom, a kitchenette and a lockable door on the unit entrance. Units are single or double occupancy.

Jersey Assistance to Community Caregivers (JACC) JACC is a State funded program that provides in-home services to seniors at risk for placement in a nursing facility.  JACC includes an array of services designed to supplement assistance given by the individual's caregiver network of family, friends, and neighbors.  By designing service plans uniquely tailored to the individual, JACC works to enhance the individual's community care options. JACC also strengthens the ability of caregivers to continue in their vital role as primary support provider.

    JACC Eligibility Criteria:
  • A New Jersey resident age 60 and older
  • Requires a nursing facility level of care but wishes to remain at home
  • Is not financial eligible for Medicaid or Medicaid Waiver Services
  • Has resources at or below $40,000 for an individual, or $60,000 for a couple
  • US Citizen or a Qualified Alien
  • Is willing and able to share in the cost of services
  • Has no alternate means to obtain the needed support
